I hereby authorise Sadie to perform upon myself cosmetic treatment either Eyelash extensions, Henna brows, Brow lamination, waxing, threading or all of above. If any unforeseen conditions arise during the course of the procedure(s) I further request and authorise her to use her full judgement and do whatever she deems advisable and necessary in the circumstances.

I understand that if I am under the age of 18 I must have a parent or guardians permission to go ahead with treatment.

I understand that NEW CLIENTS MUST attend for a compulsory skin test no less than 48 hours before ANY NEW Appointment or Treatment. This is your responsibility to attend within opening hours to have this compulsory skin test carried out (opening hours can vary so please message Sadie directly via email or Instagram to arrange this). If you do not attend within the required timescales (min 48 hours before ANY treatment.) Your Appointments can be cancelled without further notice and any booking fees forfeited.

I understand that a sensitivity test for pigment does not guarantee that I will not have an allergic response. I am aware that allergic reactions to pigment are rare and therefore accept all responsibility if an allergic response does occur.

I am aware that a sensitivity reaction to glues, wax or pigments can occur and therefore accept all responsibility if a sensitivity reaction does occur.

I accept and understand that KEEPYOUPRETTY and RD Hair & beauty salon meets the highest hygiene standards, and that sterile disposable needles are used for each individual client, procedure and visit.

 

I am aware that the result of the procedure is determined by the following factors:

·      Medication that I am currently taking and/or have taken in the past.

·      Skin characteristics – i.e. dry/oily/sun-damaged.

·      Natural skin undertones.

·      General stress.

·      Hair growth.

·      Skin conditions

·      A poor diet.

·      Post procedure care treatment.

I understand that due to the high quality of products Sadie uses, Sadie will not infill another technicians work. A removal and full set would be required in this instance.

I understand that due to my insurance, children are not allowed in treatment room. Also, because of the size of my lash room, unless prearranged, friends and family members are not permitted during your appointment.

I understand that Henna brows is NOT suitable for those with sparse hair growth.

I understand that Henna can NOT be used with brow lamination treatment.

I understand during my lash appointment I must remove my contact lenses.

I understand that If booking in for Lash Extensions and Brows on the same day, Please ensure you schedule your Lash appointment first so as not to interrupt your Brow Results.

Cancellation Policy

Failure to attend a scheduled appointment and failure to give 2 days’ notice of a cancellation or rearrangement of an appointment will waive your rights to using your deposit towards a different appointment. In addition. DEPOSITS ARE NON REFUNDABLE. SADIEKEEPYOUPRETTY reserves the right to charge 50% of the procedure as a cancellation fee.
